Ayushman Bharat Explained: How 550 Million Indians Can Get Free Healthcare (And Don't Know It Yet)

Ayushman Bharat—Pradhan Mantri Jan Arogya Yojana (PM-JAY) is the world's largest government health insurance scheme, covering 550+ million Indians (bottom 40% economically). Yet millions of eligible families don't know they're covered or how to use it. Understanding Ayushman Bharat can make healthcare free for tens of millions of Indian families.


What is Ayushman Bharat?

Launched in 2018, Ayushman Bharat provides:

Who is Eligible?

Eligibility based on 2011 Socio-Economic Caste Census (SECC) data. Approximately 107 million poor and vulnerable families (550+ million people) are eligible.


Automatic Eligibility (Rural):

Families with:

- Only one room with kucha walls and roof

- No adult member aged 16-59

- Female head of household with no adult male member

- Disabled member and no able-bodied adult

- SC/ST families

- Landless laborers deriving major income from manual labor


Automatic Eligibility (Urban):

Families engaged in:

- Rag picking

- Begging

- Domestic work

- Street vending

- Construction work

- Plumbing, electrical work

- Security guard, gardener, sweeper

- Transport workers, conductors

- Shop workers, assistants, peons

- Mechanics, washermen, tailors

If your family was identified in SECC 2011 under these categories, you're automatically eligible. Many don't realize they qualify.


How to Check Eligibility

Method 1: Online Check

Enter mobile number or ration card number or SECC 2011 name

System shows eligibility status immediately


Method 2: Ayushman Mitra at Hospitals

All empaneled hospitals have designated Ayushman Mitra (helper). They can check eligibility instantly using your Aadhaar or name.


Method 3: Common Service Centers

Visit nearest Common Service Center (CSC) with Aadhaar card. They check eligibility and issue Ayushman cards for small fee (₹20-30).


Method 4: Call Toll-Free Number

Call 14555 or 1800-111-565. Operators check eligibility over phone.


What Ayushman Bharat Covers

1,600+ procedures across 25 specialties:

- Cardiac surgery (bypass, valve replacement, angioplasty)

- Neurosurgery (brain surgery, spine surgery)

- Cancer treatment (surgery, chemotherapy, radiation)

- Organ transplants (kidney, liver, heart)

- Joint replacements (knee, hip)

- Maternity care and C-section delivery

- Pediatric surgeries

- Dialysis

- Eye surgeries (cataract, glaucoma)

- All major and minor surgeries

- ICU and ventilator costs

- Diagnostic tests related to hospitalization

- Medications during hospitalization

- Pre-hospitalization (3 days before)

- Post-hospitalization (15 days after)


What's NOT Covered:

- Outpatient department (OPD) consultations

- Routine medications for chronic diseases

- Cosmetic procedures

- Fertility treatments

- Organ donor expenses (in transplants)



How to Use Ayushman Bharat

Step 1: Verify Eligibility and Get Card

Check online or visit the Common Service Center. If eligible, get a physical Ayushman card (₹20-30) or use a virtual e-card.


Step 2: Choose an Empaneled Hospital

Find empaneled hospitals near you:


- Shows all empaneled hospitals by location and specialty

- Includes government and private hospitals

Both government and private hospitals are empaneled. Many top private hospitals participate.


Step 3: Visit the Hospital with Documents

Required documents:

- Ayushman card (physical or e-card)

- Aadhaar card

- Ration card (if available)

- Any medical records/prescriptions


Step 4: Ayushman Mitra Processes Admission

Every empaneled hospital has Ayushman Mitra (coordinator). They:

- Verify your eligibility

- Check if the procedure is covered

- Process pre-authorization

- Arrange cashless admission


Step 5: Receive Treatment - No Payment

Entire hospitalization is cashless:

- No admission deposits

- No payment for surgery, tests, medications

- No discharge payment

- Hospital bills are directly sent to Ayushman Bharat


Common Problems and Solutions


Problem 1: Hospital Says You're Not Eligible

Solution: Ask Ayushman Mitra to check again. System errors are common. If still showing ineligible but you believe you qualify, file a grievance:

- Call 14555

- Visit the district Ayushman Bharat office

- Lodge a complaint on PMJAY website


Problem 2: Hospital Asking for Money

Ayushman treatment should be 100% cashless. If the hospital demands payment:

- Refuse payment

- Contact Ayushman helpline: 14555

- File a complaint against the hospital

- Ask to speak with the hospital Ayushman coordinator

Problem 3: Procedure Denied as 'Not Covered'

Some hospitals incorrectly claim procedures aren't covered. Check official package list on PMJAY website. If the procedure is listed but the hospital denies:



Problem 4: Can't Get Ayushman Card

Physical card isn't mandatory. Treatment can proceed with:

- E-card (download from the PMJAY website after verification)

- Just Aadhaar if family is in the beneficiary database

Many assume 'free government scheme' means poor quality. Reality: Ayushman empanels hospitals meeting strict quality criteria.

Quality standards:

- NABH accreditation preferred

- Minimum infrastructure requirements

- Qualified specialist doctors are mandatory

Treatment quality is identical to paid treatment at the same hospital. Ayushman patients receive the same doctors, facilities, and care.


Ayushman + Other Insurance

If you have both Ayushman Bharat and private insurance:

- Use Ayushman first (it's free)

- Private insurance can cover expenses beyond the ₹5 lakh limit

- Private insurance can cover post-discharge medications

- Maximize Ayushman to preserve the private insurance sum insured


Portability Across States

Massive advantage: Ayushman works anywhere in India.

Aruna (name changed for anonymity) lives in Bihar, eligible under Ayushman. She needed complex neurosurgery available at AIIMS Delhi. She traveled to Delhi, was admitted to AIIMS under Ayushman, and received world-class surgery at no cost. Ayushman covered everything, including pre- and post-hospitalization expenses.
